CYO Region 19 Softball
	Meeting Schedule: League coordination meetings are scheduled as follows:
	Forms, Data Sheets and Affidavits: CYO Coach's Affidavit: The attached CYO Coach's Affidavit (PDF version.) or (Microsoft Word document version.) must be completed for each team. Once completed the form should be turned-in with the team roster. Team Data Sheets: The attached Team Data Sheet (PDF version.) or (Microsoft Word document version.) can be used to record the field availability and no play dates for each team. Once completed the form must be turned-in via email by March 5.
	Team Registration Fees: The team registration fees are as follows: Region Teams - $190 per team. Parish Teams - $190 per team. Novice Teams - $100 per team.
