Soccer Coordinators and Coaches (Limited access area) Parish Soccer Coordinators and Coaches - access limited to Region 19 Soccer Coordinators

	Soccer Camps and Coaches Clinics:
	Forms, Data Sheets and Affidavits: Team Roster: The attached Team Roster Spreadsheet (Microsoft Excel version.) must be completed for each CYO team. Players must be listed in alphabetic order by last name. Once completed the rosters should be turned-in to the appropriate parish coordinator. The parish coordinator gets the rosters signed and sealed by the parish Athletic Director and priest moderator or Pastor. They are then turned in to the league commissioner. (Instructions for completing the roster are on the first page of the spreadsheet.) CYO Coach's Affidavit: The attached CYO Coach's Affidavit (PDF version.) or (Microsoft Word document version.) must be completed for each team. Once completed the form should be turned-in at the September 1 meeting. Referee Evaluation Form: The attached Referee Evaluation Form (PDF version.) or Microsoft Word document version) can be used to record the evaluation of all participants and spectators at games. Once completed the form should be turned-in to the sport commissioner. Team Data Sheets: The attached Team Data Sheet (PDF version.) or (Microsoft Word document version.) can be used to record the field availability and no play dates for each team. Once completed the form should be turned-in at the August 1 meeting.
	Team Registration Fees: The team registration fees are as follows: Varsity Teams - $150 to AOP (AOP pulls from Parish account) + $85 to Region 19 per team. JV Teams - $150 to AOP (AOP pulls from Parish account) + $85 to Region 19 per team. Novice Teams - $60.00 to Region 19 per team.

	Referee Fees: The referee fees are as follows: Varsity Teams - $60.00 per single game - $60.00 per game for multiple games. *** JV Teams - $60.00 per single game - $60.00 per game for multiple games. *** Novice Teams - Determined by the parish providing the referee. *** *** Varsity, JV and Novice - The Home team is responsible for paying the referee. ***
