Forms, Data Sheets and Affidavits:
| Team Roster: The attached Team Roster Spreadsheet (Microsoft Excel version.) must be completed for each CYO team. Players must be listed in alphabetic order by last name. Once completed the rosters should be turned-in to the appropriate parish coordinator. The parish coordinator gets the rosters signed and sealed by the parish Athletic Director and priest moderator or Pastor. They are then turned in to the league commissioner. (Instructions for completing the roster are on the first page of the spreadsheet.) Due Aug 28th 2024. |
| CYO Coach's Affidavit: The attached CYO Coach's Affidavit (PDF version.) or (Microsoft Word document version.) must be completed for each team. Once completed the form should be turned-in with the team roster and should be signed and sealed. Due Aug 28th 2024. |

Team Registration Fees: The team registration fees are due on Aug 28th, 2024. Are as follows:
| Varsity Teams - Region 19 fee = $80, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $230] |
| JV Teams - Region 19 fee = $80, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $230]. |
Referee Fees: The referee fees are as follows:
