	Tournaments: All teams participating in a tournament must report this information to Brendan Zaba prior to participation in the tournament. brzaba@gmail.com Any team that does not report a tournament will forfeit a number of league games equal to the number of tournament games played.
	Forms, Data Sheets and Affidavits: Team Roster: The attached Team Roster Spreadsheet (Microsoft Excel version.) must be completed for each CYO team. Players must be listed in alphabetic order by last name. Once completed the rosters should be turned-in to the appropriate parish coordinator. The parish coordinator gets the rosters signed and sealed by the parish Athletic Director and priest moderator or Pastor. They are then turned in to the league commissioner. (Instructions for completing the roster are on the first page of the spreadsheet.) Player Eligibility Affidavit: The attached Player Eligibility Affidavit (PDF version.) or (Microsoft Word document version.) must be completed for each High School CYO player. Once completed the form should be turned-in at the TBD meeting. CYO Coach's Affidavit: The attached CYO Coach's Affidavit (PDF version.) or (Microsoft Word document version.) must be completed for each team. Once completed the form should be turned-in with the team roster. Team Data Sheets: The attached Team Data Sheet (PDF version.) or (Microsoft Word document version.) can be used to record the gym availability and no play dates for each team. Once completed the form should be turned-in at the October 24th meeting. Blackout Dates: Feast of All Saints; Immaculate Conception; Christmas Eve; Christmas Day; Solemnity of the Holy Mother of God.
	Team Registration Fees: The team registration fees are as follows: High School Teams - Region 19 fee = $110, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $260] Region Teams - Region 19 fee = $110, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $260] Varsity Parish Teams - Region 19 fee = $85, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $235] JV Parish Teams - Region 19 fee = $85, Archdiocesan fee = $150 per team (AOP fee pulled from parish account.) [total = $235] Junior Teams - Region 19 fee = $85
	Referee Fees: The referee fees are as follows: High School games - $50 per referee. JV and Varsity Region games - $40 per referee. Parish games using two referees(Varsity) - $35 per referee. Parish games using two referees(JV) - $30 per referee. Parish games using two referees(junior only) - $30 per referee.
